Ability to metabolize tamoxifen affects breast cancer outcomes

Dec. 26, 2012 ? For nearly a decade, breast cancer researchers studying the hormone therapy tamoxifen have been divided as to whether genetic differences in a liver enzyme affect the drug's effectiveness and the likelihood breast cancer will recur. A new study by researchers from the Mayo Clinic Cancer Center and the Austrian Breast and Colorectal Cancer Study Group provides evidence that genetic differences in the enzyme CYP2D6 play a key role in how well tamoxifen works.

"Our findings confirm that, in early breast cancer treated with tamoxifen, genetic alterations in CYP2D6 lead to a higher likelihood of recurrence and death," says Mayo Clinic oncologist Matthew Goetz, M.D., lead author of the study in the journal Clinical Cancer Research.

In the clinical trial, Dr. Goetz and his colleagues studied the rates of cancer recurrence and death in two groups: postmenopausal women with primary estrogen receptor-positive breast cancer who received tamoxifen for five years and those who received tamoxifen for two years followed by the aromatase inhibitor anastrozole for three years. Anastrozole is a breast cancer drug whose metabolism does not require the CYP2D6 enzyme.

The study showed that women who were born with genetic alterations of CYP2D6 that abolish the enzyme's critical metabolizing activity and who took tamoxifen for five years had recurrence of breast cancer, or died at a rate 2.5 times higher than women with normal CYP2D6 enzyme activity. Women with intermediate levels of the CYP2D6 enzyme had rates of recurrence or death 1.7 times higher than women with normal CYP2D6 activity. Importantly, Dr. Goetz notes, that genetic alterations in CYP2D6 did not affect the likelihood of recurrence or death in women who switched to anastrozole after two years of tamoxifen.

"Switching from tamoxifen to an aromatase inhibitor may be one reason for the discrepant studies surrounding CYP2D6 and tamoxifen -- as information about whether a patient took an aromatase inhibitor after tamoxifen was not available in most of the prior studies," says senior author James Ingle, M.D., of Mayo Clinic, an expert on hormone therapies for breast cancer.

A blood test can determine whether a woman has alterations in CYP2D6 and predict how efficiently her body will convert tamoxifen to endoxifen. Approximately 5 to 7 percent of European and North American populations are considered poor metabolizers of tamoxifen.

"The results of this successful high-level international research collaboration are an important step forward in our quest to individualize breast cancer treatment and provide tailored care to women with breast cancer," says Michael Gnant, M.D., professor of surgery at the Medical University of Vienna and president of the Austrian study group.

So what should a woman do if she is unable to effectively metabolize tamoxifen into its most active form? Dr. Goetz believes that the current recommendation of switching from tamoxifen to an aromatase inhibitor is likely to result in the greatest benefit in women with decreased CYP2D6 metabolism. For CYP2D6 poor metabolizers, avoiding tamoxifen altogether and starting out with an aromatase inhibitor may be the best approach, he says.

Dr. Goetz's group is working with the National Cancer Institute to develop endoxifen as an alternative to tamoxifen. If women can be given endoxifen, the active part of tamoxifen, it won't matter how tamoxifen gets metabolized, he says.

Samsung has just put out a fresh ad for the Galaxy Note 2 targeting enterprise users. In the span of a minute, it does a great job of showing how the Note 2 can be used to take notes, watch video, multitask, and transfer files over NFC. Despite being about work, it still has the same sense of humor as Samsung's previous TV spots, and I'm tempted to agree with the top-voted YouTube commenter for the ad: "Apple is going to have to get more creative with their marketing. A research team at the University of Exeter has used new technology to discover novel mutations which cause hyperinsulinism

Congenital hyperinsulinism is a genetic condition where a baby's pancreas secretes too much insulin. It affects approximately one in 50,000 live births and in severe cases requires the surgical removal of all or part of the pancreas.

Researchers at the University of Exeter Medical School are the first in the world to utilise new genetic sequencing technology to sequence the entirety of a gene in order to identify mutations that cause hyperinsulinism. Previously, existing technology limited such sequencing to only part of the coding regions of the gene which meant that some mutations were missed.

Using new Illumina genetic sequencing technology, the research team led by Professor Sian Ellard has discovered novel mutations that cause hyperinsulinism. Their findings are published today, 27th December 2012, on-line by The American Journal of Human Genetics.

The outcome will be that some infants born with hyperinsulinism will require fewer investigations, because the new technology means that for many only one genetic test will be required to determine the extent of the condition in each child. It also means that clinicians will have more information at their fingertips to inform them about how much of the pancreas needs to be removed.

Approximately 50 per cent of patients with congenital hyperinsulinism require surgery, and of those half require the entire pancreas to be removed. Removal of the entire pancreas increases the risk of diabetes later in life, but if left undiagnosed and untreated hyperinsulinism can result in irreparable brain damage. Symptoms range from shakiness and tiredness to seizure and coma.

Dr. Sarah Flanagan, Research Fellow in Molecular Genetics at the University of Exeter Medical School said: "The potential provided by this new technology is important and exciting, because it allows us to investigate genetic coding in its entirety. This means that investigators can identify mutations that sit at the heart of any number of conditions where before they might have been missed. This in turn results in better information for clinicians upon which they can base effective treatments and interventions for their patients."

The hormones in your body are regulated by the foods you consume, and the correct ratio of carbohydrates, fat and protein are essential for burning fat and losing weight. To keep your blood sugars steady throughout the day, eat at least four small meals that incorporate the right proportion of ?fats, carbohydrates and fats.

A healthy and balanced diet does not mean you have to give up your favorite foods. Use portion control and limit the occasions you consume foods higher in saturated fat or calories.? The right combination of protein, carbohydrates and healthy fats is an essential part of maintaining a healthy weight and building muscles:

Protein is essential after a vigorous workout.? It rebuilds muscle; protein is the building material for muscle tissue.? Protein aids in repairing and rebuilding muscle tissue that is broken down during strenuous exercise. Aim for 40% of your daily calories from lean protein sources.? Load up on lean protein sources such as steak, chicken, nuts, egg whites, salmon or other seafood, and lentils.? Try to put the majority of your lean protein sources into a lower fat content protein than lower grade beef or pork. Merry Christmas! The USPTO is celebrating in a big way, with the copyright stamp making some pretty big rounds today. First up is a design patent for an iPod touch, which Cupertino filed back in August of 2011. It looks to be the fourth-gen model from 2010, especially given that the patent focuses on the very rounded edges. This iPod touch was the first version to include both front- and rear-facing cameras, and in any case, the design is miles thicker and shorter than this year's touch. As Patently Apple points out, this document also happens to be one of the last to list Steve Jobs as an inventor.

Thanks to cheap editing software and a prevailing geek ethic, virals featuring comic book characters/movie franchise heroes (Batman Gets Lost Thanks to Apple Maps) and straight-up Pixar favorites (?Toy Story? toys burn up in family prank on Mom) were among the most popular this year. And let?s not forget the mashups: Prepare for your holiday travel with the Scariest Flights from Films in One Supercut and then calm your nerves by watching 250 Movies in 2 Minutes.

Downton Tabby

Popular TV shows were ripe for parody. PBS?s ?Downton Abbey? offered the most fertile ground for hilarity, from the use of cats (see above) to subbing in a former Will Smith character (The Fresh Prince of ?Downton Abbey?). ?Breaking Bad? also was popular for mixologists, who blended the meth-making characters with a pop tune (Taylor Swift Takes ?Breaking Bad? Turn). Sometimes multiple shows got mashed together, as in Don Draper Eats Brains on ?Walking Dead Mad Men.? And wouldn?t ?Game of Thrones? be even more interesting if, instead of swords, they borrowed from "Star Wars"? (?Game of Thrones? with Lightsabers.)

Happy Little Trees Grow Again in Bob Ross video

If ?Downton Abbey? held a warm place in many readers? hearts, nostalgia for PBS and Muppet-related programming was white-hot. Gentle painter Bob Ross was able to paint again, while singing, thanks to auto-tune (see video above), a connection was forged between Cookie Monster and Batman (Video Proves Dark Knight is Secretly Cookie Monster), and the Muppets welcomed ?The Voice?s? U.S. Naval Academy Midshipmen Go ?Gangnam Style?

A newcomer to the U.S. must think everyone is out there dancing (or galloping) in the streets 24/7, thanks to the flood of ?Gangnam Style? and ?Call Me Maybe? tribute videos. (The original "Gangnam Style" video itself hit 1 billion views on Dec. 21.) Among the most-watched of these videos here at TODAY.com were the Olympic Swimming Team?s tribute to the Carly Rae Jepsen song?and the Naval Academy?s take on ?Gangnam Style.? Ohio University?s band also got into the ?Gangnam? act, as did Hugh Jackman when singer PSY visited the ?Wolverine? set, and clever editors managed to get both Klingons and Bill Nye in on the Korean pop sensation. From the beginning, there have been creatures, both dangerous and friendly. And there have been those who have been chosen to defend those who matter and annihilate those who pose a threat. To keep the world in balance, there must be those who protect secrets. They are called The Keeper?s; they are the ones who must conserve the second world. The Ruinosus have been around since the dawn of time, trying their hardest to take what they think is rightfully theirs, the world, and turn it into a surface hell. Though they are strong, the Keepers are stronger. From Mongolian Deathworms to Shadow People, from The Four Dragon Kings to the Drac of Romania, there are many different types of monsters, both weak and strong. Different monsters have different holds on the world, to keep it balanced and well. But if they are influenced in the wrong way, their grips will loosen and all with fall, inevitably bringing an end to the world. Not all creatures are under the influence of the greater good, and are under the black wings of the Ruinosus. Under the surface of the earth, there is a battle waging back and forth between the dark and the light, between Keeper's and Ruinosus', between order and ruin. The Ruinosus have a way of persuading, and now they are getting more and more over to their side, to come forth and fight. The Keepers are up for a challenge they might not be able to face on their own.
At a modern high school, Henry Jefferson High, there is a club. But this isn?t your average club. It?s a high-class, exclusive magical arts club, where only those with real magical skills are allowed. This is where they train the protectors of the world; the kids who stop the supernatural, and not let it be known to the outside world.

But now, all their training is put to an early test, as the students are sent out into the world, to protect it from the beasts that lie beneath the surface and above it.

Lately, disappearances and strange events, or phenomena, have been occurring throughout the world. The world?s great leaders have concluded that they must call on the help of the young students of Henry Jefferson High?s Magical Arts Club to control, destroy, or capture the beast?s roaming on the earth. With innocent lives at stake, they cannot refuse. Now these teenagers have to use their abilities to keep their friends, family and their secret safe.

The Roles

The Keeper's:

As a child, a keeper is taught to hone in their powers. They don't really gain their full powers until they reach teenage years. Powers have been learned, but not fully controlled, not until they reach adulthood, about the age of 27. No teen has perfectly mastered their powers; they still go bizarre on occasions. Some teens can't even fully use their powers yet. There are 8 basic power areas: wind, earth, water, ice, air, fire, shadow and light. Keepers have skills in one specific area, such as water or fire. But mentors teach them minor spells and skills in the other areas. Most Keepers have one skill that wipe's them out of energy, and a few minor things they can do.

Example: Clara Johanna, Power's: Fire: Can summon fire at will, start fires with mind up to 300 ft. away. Special Attack: Fire Whirlwind.

A special attack is something that you are taught to use only when you really need to. Using it takes a ton of energy from your body, so much that you are knocked out on the spot, no matter what. A person, depending on their rank, is knocked out for a few days to recharge their powers. Every one of the students attending starts at U-Q rank, but the speed of ranks which you move up is all decided by your instructor.

Ranks:
V-Z: Up to 6-5 days to recharge.
U-Q: Up to 5-4 days to recharge.
P-L: Up to 4-3 days to recharge.
K-G: Up to 3-2 days to recharge.
F-B: Up to 2-1 days to recharge.

Your Demons:

Every kid in the Magic Arts Club has a demon. They have had their demon ever since they were little. Only those with magical abilities can see demons. The demons aren't bigger than their owners until they graduate high school. You demon must be smallish in size. Your demon can have skills in a specific area, such as shadow, fire, ice, wind or water. There are many other areas, but they are usually the same area of skill as their masters. Your demon can be humanoid in looks or more animal like. It can be totally unique or totally generic, it's up to you. Nothing is impossible, as long as they are smallish in size, they should be fine. If you can't find a picture, a good description is fine. Your demon will be taking a different class than you, do make it to where, "My demon will die if it is not near me for more than 2 minutes."

School Rules
1. Girls and boys aren?t allowed in each other?s dorms. (I.e. girls in girl?s dorm, boys in boy?s dorm)
2. Must attend breakfast, lunch and dinner.
3. Cannot be outside of school grounds after 8:00 p.m.
4. Must be in bed by 9:00 p.m.
5. Fighting. Bullying, drugs, alcohol or anything related to these is prohibited.
6. Don?t wander down forbidden halls or rooms, they are forbidden for a reason.
(You don't have to follow them, but if a teacher catches you, you?re screwed.)

Magical Club Rules

1. No magic outside of classroom, unless supervised.
2. You must never speak of what the real club does. (Cover up is the Medieval Club.)
3. Must stay with your group or partner at all times if on a mission or just on field trip.
4. Who you really are and what you do is not to be made known to outsiders.
5. You must obey direct orders from higher ranks than you.
6. Mr. Bekenfield is your instructor, but that doesn?t mean he is a sore loser. Be free to talk to him about any troubles or questions you may have.

Make sure you spread out your abilities. Not everyone has to be fire or air or shadow. If you have read this entire intro, please put your favorite food somewhere in your history.

There will be school trips around the world occasionally, unless otherwise notified. A villain will be later added, so if you want to reserve that, you are welcome to. I will explain things then.

The Chosen Ones
Humans with a hand-given destiny by Arceus to save the world.

The Dreams
Since they were small children, they have had strange dreams. Dreams that were nothing more than fleeting glimpses of colorful and glowing shardsImage and even the Sword of Vale. They've had visions of Arceus and a strange world where there are creatures that resemble Pokemon, but none of the Pokemon that they've ever seen.

Team Mystic
The "evil" side of the war, apparently. They want to capture The Chosen Ones to help them recruit Pokemon and find Arceus' treasures.

The War
Team Mystic has been using their Pok?mon to abuse humans and mutants alike, greedy for rare Pok?mon and treasures. They've recently discovered information regarding the Mystic Shards and The Chosen Ones and now seek to possess them. Their abuse has grown as they stop at nothing to find these Chosen Ones.

Arceus Treasures
The very last shards, larger than the rest. Its rumored that they can create new Pok?mon and even open doors to other dimensions. Hidden by Arceus - Only the Chosen Ones have an idea of where they could be. They ofen have dreams showing fleeting images of the shards' whereabouts. There is also the Sword of the Vale of which Team Mystic wishes to use to control the Dragon Force. No-one knows where it is now, but the Chosen Ones often see fleeting glimpses of it in their dreams just as much as the Mystic Shards.

Pokemon
The Chosen Ones can only capture Pokemon that correspond with their Type. For now. They have an intense bond with Pokemon of their Type and literally have never felt the need to capture a Pokemon of any other Type. The ability to capture other Pokemon Types will be introduced soon - With a light twist. What happens if they try to capture a Pokemon not their Type, you ask? Their Pokeball simply won't work at all. What happens if their Pokemon will evolve into something that isn't their corresponding type anymore? They won't evolve. The trainer can choose to let it evolve, but they will no longer own it - The Ownership seal from the Pokeball will break.

Communication
All Chosen Ones can use telepathy with their Pok?mon and Pokemon that they've established a strong bond with. Chosen Ones can understand all Pok?mon when they speak - Not just their own. They can also feel their Pokemon's emotions as their Pokemon can feel their trainer's as well.
